Chronic obstructive pulmonary disease (COPD) and long COVID represent chronic conditions that significantly affect patients’ health and quality of life. Along with medical factors, non-medical risks are becoming increasingly important: social isolation, mental and cognitive disorders, advanced age, limited access to medical care and digital technologies, and low self-care ability. These factors reduce adherence to therapy, complicate the implementation of medical recommendations, and contribute to adverse outcomes. This paper analyzes their role in patient management and proposes strategies for a comprehensive approach that takes into account the social determinants of health and the biopsychosocial model of care.
