Abstract

Introduction. Macrolides are essential chemotherapeutic agents with bacteriostatic and bactericidal effects in the treatment of chronic inflammatory conditions. This article explores their mechanisms of action and therapeutic potential in diseases such as non-cystic fibrosis bronchiectasis, asthma, and diffuse panbronchiolitis. Materials and Methods. To evaluate the role of macrolides in the treatment of chronic inflammatory diseases, a systematic literature review was conducted using databases such as PubMed Central, Cochrane Library, Medline, and Google Scholar. The studies included were selected based on relevance and quality, with a particular focus on the mechanisms of action described in the context of the studied conditions. Results. The analysis of the articles demonstrated that macrolides exert various effects on the immune system and the inflammatory process. These include, but are not limited to, modulation of the immune response, inhibition of pro-inflammatory cytokine production, and reduction of pathogen activity through both direct and indirect protective mechanisms. Conclusions. Macrolides prove to be valuable tools in the management of chronic inflammatory diseases, offering not only an antimicrobial effect but also anti-inflammatory propertie s that can improve patients’ quality of life. This synthesis highlights the need for further research to optimize the use of macrolides in clinical practice.
