UTILIZAREA CRITERIILOR PENTRU TROMBOZELE INTRACEREBRALE POST- COVID-19: SERIE DE CAZURI

Rezumat

Diagnosticul trombozei venoase cerebrale (TVC) post-COVID-19 la maturi se efectuează conform criteriilor: (1) vârsta ≥18 ani; (2) TVC confirmată imagistic sau la autopsie; (3) COVID-19 confirmat prin test PCR cu 90 zile înainte de diagnosticare TVC sau în timpul internării.Scopul lucrării: Analiza utilității criteriilor de diagnostic pentru stabilirea diagnosticului de tromboză cerebrală post- COVID-19. Material și Metode: Au fost evaluate cazurile tuturor pacienților suspecți de TVC, internați în Institutul de Neurologie și Neurochirurgie „Diomid Gherman” în anii 2020 - 2023. A fost analizată aplicarea criteriilor diagnostice. Rezultate și discuții: Au fost identificați 5 pacienți cu vârsta 30-66 ani, diagnosticați cu TVC post-COVID-19 confirmat prin test PCR. Acuze prezentate la internare au fost: cefalee severă instalată brusc, diplopie, dureri retroorbitale, exoftalm, ataxie. Investigații efectuate au fost: angio-tomografie computerizată (CT), imagistică prin rezonanță magnetică (IRM) cerebrală cu contrast, angiografie cu substracţie digitală a capului, angiografie clasică. TVC a fost determinată la nivelul sinusului transvers și cavernos. Timpul mediu de la confirmarea COVID-19 la debut TVC a fost 93 zile. Dintre acești pacienți, unul a prezentat AVC de tip ischemic cu transformare hemoragică în sistemul vertebro-bazilar, utilizator de contraceptive orale, în antecedente cu migrenă cu aură; altul a prezentat meningită secundară bacteriană. A fost inițiat tratament anticoagulant, unor pacienți suplimentar antibiotice. Concluzii. Tromboza venoasă cerebrală trebuie suspectată în cazurile tuturor pacienților post-COVID-19. Utilizarea criteriilor de diagnostic poate facilita diagnosticul precoce și abordarea terapeutică promptă și corectă a pacienților.
